CONSENT
You are invited to participate in an online evaluation for the 2025 SMC Faculty Fellows Program, conducted by Chitra Productions. Completing the evaluation should take about 15-20 minutes if you are partnered with one fellow. If you are working with more fellows, please allow an extra five minutes for each additional participant. The aim of this evaluation is to collect feedback that will help the team ensure the project’s outcomes can be replicated. The information gathered will be analyzed to assess the program’s effectiveness and educational value.

PARTICIPATION
All partners participating in the Fellowship Program are asked to complete the first part of the evaluation and the portion pertaining to the fellows for each individual.

BENEFITS
Participating in this evaluation will not directly benefit you. However, your responses may help us refine the program to better support you and your fellows.

RISKS
There are no foreseeable risks associated with completing this evaluation beyond those encountered in everyday life. The data collected will be used solely for evaluation purposes, and no physical, psychological, or social risks have been identified.

CONFIDENTIALITY
Your evaluation responses will be submitted via an online form and stored in a password-protected electronic format. The data will be reported anonymously, ensuring that no names will be included in any reports generated by the elevators.
