CONSENT
You are invited to participate in an online evaluation for the 2025 Senior Capstone Immersion Program conducted by Chitra Productions. Completing the evaluation should take about 15-20 minutes. The aim of this evaluation is to collect feedback that will help the team ensure the project’s outcomes can be replicated. The information gathered will be analyzed to assess the program’s effectiveness and educational value.

PARTICIPATION
All student researchers participating in the student research program are asked to complete the evaluation.

BENEFITS
You will not receive any direct benefits from participating in this evaluation. However, your responses may assist us in refining the program to better support future students in the research program.

RISKS
There are no foreseeable risks associated with completing this evaluation beyond those encountered in everyday life. The data collected will be used solely for evaluation purposes, and no physical, psychological, or social risks have been identified.

CONFIDENTIALITY
Your evaluation responses will be submitted via an online form and stored in a password-protected electronic format. The data will be reported anonymously, ensuring that no names will be included in any reports generated by the elevators.
